2. A Copy of intended travel itinerary or confirmed booking
3. Copy of Passport Bio page
4. Copy of Work Permit/Resident Permit
5. Proof of 14-Days Hotel Payment Receipt for Quarantine See all Hotels Here
6. A Request letter addressed to the Controller
7. Provide copy of COVID-19 Test Certification
